Admissions Application

Application materials are available below. In addition, some forms (as noted) may be submitted online. Please review the admission requirements and the admission procedures and deadlines prior to submitting your application materials.

To submit your application materials, please note the following procedures:

1. This completed application and ALL other required application documents can be received by post in hard copy or on a CD, e-mail with attachments, or fax. Additionally, some of the application materials can be completed on-line. For those sending by post, the address is:

2. Request that official transcripts of credit be sent directly from each college or university, seminary and/or graduate school you have attended to the Office of Admissions.

3. Request that your letters of reference be sent directly from each referent to the Office of Admissions.

4.  Transcripts: please read the application procedures and deadlines instructions regarding transcripts.

5.  Letters of recommendation coversheets: download hardcopy  (you will need three copies).

6. You can download a hardcopy of the Letter of Endorsement from Ministry Site for DMin Application.

7.  For the biographical essay, statement of purpose, and work sample: You may either mail these items to the Office of Admissions with your other materials, send them as This e-mail address is being protected from spambots. 8.  $75 non-refundable application fee: Mail your check or money order to the office of admissions. Please be sure to include your full name and social security number (if applicable)
